• Photo Permission Slip

    This permission slip gives the AmeriCorps Member permission to post pictures that were taken of you or your child(ren) during community service projects.  The picture(s)may be published in the newspaper, on AmeriCorps website (www.acstart.org,) on the AmeriCorps page located under Community on ALCS's website, or publically displayed around the high school.

    If you agree to allow us to publish/post your or your child(ren)'s picture, please print and sign the following form and return/mail it to Mindy as soon as possible.  Thank you very much!

  • START Permission Slip

    This permission slip is to request your permission to work with your child and to allow me to gather date on your child from the school, which is necessary for me to complete certain documents.  The information I collect will be kept strictly confidential.  Please print the document, fill it out completely, and sent it in with your child.  If you have any questions or concerns, please don't ever hesitate to call or even stop by to meet with me.  Thank your for letting your child participate in the AmeriCorps program!
